This is in my chaplain’s prayerbook for attending end-of-life situations. I wrote it for that long, quiet vigil sitting with the dying, when communication with this world appears to have ceased yet life remains. Sometimes the dying seem to need permission to let go. Sometimes the family present needs to offer that permission, for their own letting go.
In any case, the dying will pass in their own time, not ours.

Brave soul, you have done in this life all that you were sent here to do, and you are free to go on to your life that is to come. Go forth in your time with our thanks and love.
We who remain hold you in our hearts and will remember you. Of all else, you are free. Go forth in your time with our thanks and love.
Rest your burdens of care beside the path; others will carry them. Go forth unencumbered with our thanks and love.
Brave soul, you leave this Earth better for all you have done in this life. Go forth with thanks and love.
Brave soul, child of Earth and starry Heaven, be at peace here and on your journey. Blessed be…at peace…beloved.
